The cause of death for heavy metal icon Ozzy Osbourne has been revealed.

According to his death certificate obtained by The New York Times, the legendary rocker died of acute myocardial infarction -- a heart attack -- and out-of-hospital cardiac arrest.

Other contributing factors included coronary artery disease and Parkinson's disease with autonomic dysfunction.

Osbourne passed away on July 22 at 76 years old.

The “Crazy Train” singer leaves behind his wife Sharon and six children: Kelly, Aimee and Jack, who he shares with Sharon, and Jessica, Louis and Elliot, who he shares with first wife Thelma Riley. He also leaves behind grandchildren.
