The Wethersfield Volunteer Fire Department is being cited for a serious safety violation following the death of one of its firefighters during a massive brush fire last year.
Wethersfield firefighter Robert Sharkevich Sr. died in the line of duty while battling the brush fire, also known as the Hawthorne Fire, on Lamentation Mountain in October last year.

Crews used a UTV while battling the fire, and it became unstable on loose terrain, causing the vehicle to roll over. Two firefighters were ejected, one of which was hit by the UTV and seriously injured, the report states.
OSHA said no seat belts or helmets were worn, and proper training wasn't conducted before the UTV was used.
