Posterior approach
It is 2025, and with modern tissue-sparing approaches I believe total hip arthroplasty is one of the most successful procedures in all of surgery, and patients will do well regardless of the approach.
I am personally still shocked we are having this debate and believe surgical approach comes down to surgeon preference and risk tolerance. Each approach has its slight advantages and disadvantages, and, ultimately, a surgeon and patient must be OK with that choice. I prefer the posterior approach for several reasons:
It is great for education. Teaching the next generation of total joint surgeons to do a well-done posterior approach is a key part of any residency or fellowship
