COLUMBUS, Ohio (WCMH) — A 3-year-old is dead, and two people, including his mother, are facing charges Monday after Columbus police say the toddler accidentally shot himself while in bed.

Officers were sent at 8:55 a.m. to Kodiak Drive, a street in a neighborhood north of Canal Winchester on the Southeast Side. They found Cartier Smith, 3, had been shot. Medics took him to a hospital where he was pronounced deceased at 9:51 a.m.

According to a court document, Sean Perrin brought the gun to the residence, where Smith's mother, Carley Dials, found it on the bedroom floor. Before going to bed about 3 a.m., she placed it between the mattress and the wall.

In the morning, Smith came into the bedroom, got into the bed, found the gun and accidentally shot himself. Dials called 911, and police
