LITTLE ROCK, Ark. (KNWA/KFTA) — State officials say Grant Hardin’s escape from a north Arkansas prison stemmed from his careful planning, and multiple staff failure, in a report spanning more than 900 pages.

Hardin, 56, a former police chief serving a multi-decade sentence for rape and murder, walked out of the North Central Unit in Calico Rock on May 25. Court documents previously said Hardin was let out by a guard, later identified as Correctional Officer William Walker, and was wearing an “ADC-type” prison uniform.

He was captured by the U.S. Border Patrol 12 days after his escape and was found around a mile and a half away from the prison.
