Fort Wayne Medical Education Program has experienced a data breach involving personal information of some patients, employees and employees’ dependents, officials announced. The suspicious activity was discovered in December.

For a limited number of employees and employees’ dependents, the information included first and last name along with a Social Security number, driver’s license or state ID number, or passport number.
