A midwife has recounted the 'absolute chaos' that unfolded during a tragic home birth, resulting in the death of both mother and baby. Vital records were hastily scribbled on an incontinence pad before being discarded. ‌

Jennifer Cahill, aged 34, tragically passed away in hospital the day after giving birth at her Prestwich home. Her baby, Agnes, died four days later from hypoxia, a condition caused by lack of oxygen, as revealed by Coroner Joanne Kearsley during an inquest held today. ‌
