The 5-year-old girl who went overboard on the Disney Dream cruise ship in June was too small to trigger the man overboard alarms, according to police

In a police report obtained by PEOPLE, investigators determined the alarm was only triggered after the girl's father jumped off the ship in an attempt to rescue her

The father and daughter were both rescued and safely returned to the ship

The incident occurred after the girl was allegedly encouraged to pose for a photo in an open porthole by her mother
