NASHVILLE, Tenn. (WKRN) -- A talent agency announced the death of a Middle Tennessee actress from "9-1-1: Nashville," less than two weeks after the show first premiered.

According to her obituary, Isabelle Adora Tate -- who "was born and raised in Nashville, TN, and grew up in the Franklin community" -- passed away on Sunday, Oct. 19, at the age of 23.

The McCray Agency reported Isabelle had a rare form of Charcot-Marie-Tooth disease, adding that she died peacefully her family requested privacy as they deal with the "sudden and shocking loss."

"I was so lucky to know her and she will be greatly missed by so many," an agency representative posted on social media.
