Health Team makes gains connecting residents to primary care

Kawartha Lakes Haliburton Ontario Health Team has made significant progress in reducing the Health Care Connect wait list by connecting hundreds to primary care.

Working with family health teams, nurse practitioner-led clinics, community health centres, and independent physician practices, the local team is helping unattached residents connect with a primary-care provider.
