It’s one of the most disturbing scandals ever uncovered in the British justice system.

Now a report into the abuse of thousands of young inmates at a notorious detention centre says it could have been prevented if those in power had taken action.

The Prisons and Probation Ombudsman Adrian Usher found a culture of systemic and extreme physical abuse at Medomsley, spanning 26 years.

Hundreds of young men were raped and sexually assaulted by one officer, leaders failed at every level.

Today for the first time, the government apologised.
