Bryan Vasquez's life and tragic death:

About three months into his life, Bryan’s nightmare began.

It was in New Orleans East in 2013. He had a skull fracture, brain swelling, broken legs and punctured lungs. His mother, Hilda, was convicted of abusing him, causing a traumatic brain injury.

"For one, he should have never been sent back home," a former Department of Children and Family Services (DCFS) worker told WDSU Investigates. "There's no accountability across the board."
