When she was led away from the railway tracks by police, Charlotte Tetley told officers she was feeling suicidal. She was taken to hospital, but left without being seen by the mental health team.
Her disappearance sparked fears among staff that the 33-year-old, who was a victim of a Rochdale grooming gang, was planning to take her own life. But when they rang 999 to report Ms Tetley as a high-risk missing person they were told no police officers would be deployed.
Despite the matter being taken up by the hospital's clinical lead of psychiatric liaison, Cheshire police still refused to attend as Ms Tetley had 'had not expressed an intention to end her life before leaving the department'.
