Twelve more UK prisoners have been accidentally released in the past three weeks, with two still at large according to Justice Secretary David Lammy.
Between April and October, 91 additional prisoners were also freed by mistake in England and Wales. Lammy told the BBC there would always be “human error” while prisons were using their paper-based system, urging patience while they wait for a “completely digital system” to be adopted which would improve things.
