LANSING, Mich. (WILX) - Healthy! Capital Counties has released its 2025–2027 Community Health Improvement Plan (CHIP), a community-driven strategy outlining measurable actions to improve health and well-being across Clinton, Eaton, and Ingham counties.
The plan translates findings from the 2024 Community Health Assessment (CHA) into targeted efforts for the next three years.
The 2025 CHIP places a strong emphasis on health equity, policy, social well-being, and long-term community action.
The plan focuses on three priority areas identified in the 2024 CHA:
Access to Care
Behavioral Health
Housing
To address these priorities, 23 community partners have committed to implementing 73 activities throughout the Tri-County region.
