The most common type of surgical error in NHS hospitals across the UK has been revealed, according to a new report analysing 'never events'.
'Never events' are defined as serious, largely preventable incidents that should not occur if healthcare providers adhere to established safety guidelines.
Fresh data uncovers hundreds of mistakes nationally across a ten-year period - including 24 claims for compensation within University Hospitals Birmingham Trust, which runs the Queen Elizabeth Hospital .
