An investigation has been launched into the death of a man crushed by machinery at the same Australian workplace where a worker was killed in similar circumstance just a few years ago.

A 45-year-old apprentice was critically injured at Hofmann Engineering in Bassendean, Perth on Monday afternoon when a steel object he was working near fell on him, WorkSafe said.

Paramedics raced to the workshop on Alice St but he could not be saved.

His employer said its “deepest sympathies and condolences” went out to the man’s loved ones and colleagues “during this difficult time”.
