The incident happened at the Forest Reach residential development in Huntley, where worker Michael Bagnato - employed by subcontractor Topdeck - suffered a fall while stripping away formwork.
Mr Bagnato was loosening bolts on the southern side of Reed Creek Bridge and was not wearing a harness, as the steel to anchor it was on the other side.
