A man died after a “brief struggle” last week with police who had arrived at a Mattapan treatment facility to remove him at the request of staff, records show.

According to a redacted police report, officers were called to a Bay Cove Human Services facility on Walk Hill Street just after 7:50 p.m. on Sept. 11 after a man was reported to be experiencing a “cocaine OD, [and] acting erratic.”

Staff wanted “him removed” from the program, the report said.

“Upon arrival, officers made contact with the victim,” the report said. “A brief struggle ensued, and officers detained the victim.”

Paramedics arrived and took the man to Boston Medical Center for further evaluation, the report said. He was pronounced dead shortly before 9 p.m .
