An investigation has found insufficient lookout arrangements on a boat which killed a Greater Manchester diver.

Paul Smith had been part of a group exploring the wreckage of SMS Markgraf, a German battleship, in Scapa Flow near Orkney when tragedy struck.

The 70-year-old was fatally hit by the propeller of a support vessel, called the Karin, on September 28, 2023.

The incident has since been investigated by the Marine Accident Investigation Branch (MAIB), which released a report on the incident today (October 16).

Mr Smith had been diving from a second workboat, the Jean Elaine, before the fatal accident took place.

At the time of the accident, Mr Smith was ascending from the wreck of the First World War
