When a San Francisco mother enrolled her daughter in the On Lok Enhanced Care Management (ECM) program, she didn’t expect it would change her life.

The woman’s adult daughter, who has a disability, hadn’t seen a doctor in seven years. Through a referral to ECM, she was connected to a lead care manager who helped her finally receive medical attention.
