More than 500 Townsville Hospital urology patients waiting for reports were "lost to the system", Queensland's health minister says.

Queensland Health Director-General David Rosengren launched an independent investigation last month when it was revealed some urology services patients had not received test results or follow-up appointments.

The investigation found 25 urology patients, some with cancer, were not progressed onto the next stage of clinical care.

Health Minister Tim Nicholls said on Wednesday there had been a failure in the delivery of critical clinical care at Townsville Hospital and Health Service.

"There was a failure to report back in respect to pathology and other testing regarding urology services at the Townsville University Hospital," Mr Nicholls said.
