CHICOPEE, Mass. (WWLP) - Chicopee Concrete Service has been fined nearly $58,000 by OSHA following a deadly workplace accident earlier this year.

Victim identified in deadly Greenfield workplace accident

The Occupational Safety and Health Administration (OSHA) imposed the fines after 66-year-old Matthew Nedorostek of Westfield died from asphyxiation when he fell into a cement hopper at a work site in Greenfield in January.

According to OSHA's report, Nedorostek was attempting to remove a blockage in the cement hopper when the accident occurred.

The investigation cited Chicopee Concrete Service for 14 violations, including failure to provide proper training and the absence of an appropriate guardrail system.

The company is currently contesting the fines imposed by OSHA.
