FLAGSTAFF, AZ (AZFamily) — A Flagstaff police officer stepped down on Tuesday after being involved in two on-duty crashes, including one that left the driver dead.

Flagstaff PD says Officer Jacob Buckwald resigned on Tuesday due to medical reasons.

On Sunday at 7:30 a.m., Buckwald was involved in a crash near Butler Avenue and Sinagua Heights Drive .

Crews immediately tried to help the other driver, but he died at the scene. Flagstaff Police say that the man has been identified, but his family is choosing not to share his name with the public.

Officer Buckwald was taken to the hospital with minor injuries and then later released. He was placed on administrative leave amid an independent investigation.
