GREENWOOD, S.C. (WSPA) -- An Upstate man has been arrested in connection with a fraud investigation in Greenwood County.

27-year-old Ziare Daquan Maurice Butler, of Greenwood, was charged with insurance fraud and forgery by the South Carolina Law Enforcement Division on Friday, September 26.

According to the arrest warrants, Butler allegedly submitted a false insurance claim to Progressive Max Insurance Company on January 7 in Greenwood County, involving an altered medical bill for $6,891.06.

The South Carolina Department of Insurance requested the investigation into Butler's activities, which led to the charges.

Butler was booked into the Greenwood County Detention Center following the charges.

The case will be prosecuted by the South Carolina Department of Insurance.
