Serious failings have been identified in the care of a tragic student nurse on an NHS ward. A coroner concluded Cerys Lupton-Jones' death at a Greater Manchester mental health facility was 'contributed to by neglect'.

He described the events prior to the 22-year-old's death as a 'shambles' and raised concerns about 'falsified' observations logs.

Cerys died on May 18, 2022, five days after she was found unconscious in a bathroom on the Elm Ward at Park House, formerly based at North Manchester General Hospital. Park House has since closed.
