Andrea Foster and her son Peter Spiniello continue to feel let down by systems they believe caused the death of their daughter and sister, seven years ago.
For them, findings released by the coroner's court in April were meant to offer some healing, but instead they said it was another missed opportunity.
The mother and son are speaking out to shine further light on what they see as ongoing shortfalls in the state's mental health and justice systems.
Michelle Foster was killed in October 2018 by then 20-year-old Jayden Tanee Lowah, who was experiencing delusions because of his schizophrenia, outside the Colonnades Shopping Centre, in Adelaide's southern suburbs.
