A Midlands mum was found dead in her bed just hours after she was sent home from A&E.
Tanya McCartney went to the accident and emergency department with a high temperature, shortness of breath and a painful throat.
The 25-year-old had first thought that she caught a cold from her son Marcus, then 11 months, but went to hospital on the advice of her GP.
Medics at the hospital failed to diagnose her pneumonia which could have been treated with a course of intravenous antibiotics.
Instead, Tanya was sent her home with oral antibiotics but her health continued to deteriorate and she was found dead in her bed the next day.
Her family demanded an investigation into her death which revealed the hospital failed to recognise the severity of her symptoms.
