The brother of a man who died after a routine operation went wrong has spoken out about the ongoing grief for his family.

Keith Williamson was just 42 when he underwent what should have been routine valve replacement surgery at Liverpool Heart and Chest Hospital in October 2015.

During the procedure, lead surgeon Darryl Chung mistakenly inserted the wrong type of valve.

An inquest into his death in 2016 found this error to be a direct result of a communication breakdown within the surgical team operating on Keith, the Liverpool Echo reported.
