Neither Cameron Goss, the shift boss at Henty Gold Mine, nor anyone in management knew there was danger underneath where the loader operator was working before his machine plunged into a void, causing his death.

In his findings released on Wednesday, coroner Simon Cooper said vital mining information had been lost due to a frequent change in ownership, periods of shutdown and the passage of time.

"All that explains, but does not excuse, the loss of knowledge and information in relation to both history and work practices," Mr Cooper said.

The company that owned Henty Gold Mine at the time, Unity Mining, was fined $150,000 for failing in its workplace safety duties.
