A “mistake” led to the death of a prisoner in a Cambridgeshire prison in 2019, a report has revealed. Jason Birch, 46, died on September 26, 2019 in Peterborough City Hospital after he was found in his cell at HMP Whitemoor in March on September 24.
Mr Birch took his own life. He was initially convicted of burglary in 2012 and sentenced to five years in prison.
In 2014, he was convicted of attacking another prisoner and sentenced to a further 12 years in prison. Mr Birch was transferred to Rampton Hospital, a high-secure psychiatric hospital, in August 2016, and then discharged to HMP Whitemoor on June 28, 2019.
