FREMONT, Neb. —

The U.S. Chemical Safety and Hazard Investigation Board has launched an investigation into the deadly explosion in Fremont.

What we know:

The explosion happened on July 29 at Horizon Biofuels Inc. in Fremont.

Dylan , Hayven , and Fayeah Danielson were killed in the incident, which was caused by an accidental dust fire, according to the State Fire Marshal's Office.

“This horrific incident should never have happened. We want to prevent a terrible tragedy like from occurring again," said CSB Chairperson Steve Owens said.
